3 Form of the reports 3 Standardised forms and entry screens are available to issuers to enable them to fulfil their regular reporting obligations. SIX Exchange Regulation provides an electronic reporting platform (Connexor Reporting) for use in connection with the regular reporting obligations set out in Annex 1 of this Circular (equity securities). See also:4 Each report must clearly reflect which issuers and securities are involved, as well as the name of the person who has submitted the report to SIX Exchange Regulation (the responsible individual, including telephone number and e-mail address in case any further information is required). In addition, it must be clearly evident which reportable fact is being addressed (indication of the corresponding Annex, including the point numbers in annexes 1 to 5).
